Cookies are not used to track personal health data and no identifying health information is collected through browsing.

Inquiring About and/or Receiving Services

The types of personal information collected in the course of communicating and/or working with you may include your/your child(ren)’s name(s), date(s) of birth, address, health history, infant feeding history, and records of the care provided to you/your children. I collect, use, and disclose personal health information for the following purposes:

  • to provide lactation assessment and care to my clients
  • to obtain payment for services provided
  • to conduct quality improvement and risk management activities
  • to comply with my regulatory obligations to the College of Dietitians of Ontario and the International Board of Lactation Consultant Examiners
  • for other purposes permitted by law

I will collect, use, and disclose only as much personal health information as needed to achieve these purposes. 

You can withhold or withdraw your consent to the collection, use, or disclosure of your personal health information by contacting me (details below).

Third-Party Services

Secure third-party service providers may be used to facilitate provision of services, to provide service on my behalf, to perform service-related services, or to assist me in analyzing how my service is used. These providers only use your personal information for the purpose of providing their services to me and are obligated to comply with Canadian privacy laws and protect your information.
